A free and thorough needs assessment, conducted by our care manager, a licensed professional nurse, is the first step in determining potential areas of concern. The assessment includes the following:

• Functional, Physical Capacity and Limitations Assessment
• Nutrition Assessment

• Community Resource Assessment

The gathering of this information will assist the care manager in fully evaluating existing services that may be in place as well as determining any unmet needs in order to develop a comprehensive set of recommendations.

Often, a client only needs assistance with daily activities. Just For You is a companionship service by Comfort Connection that offers:

Following the needs assessment and collaboration, with all interested parties, a mutually agreed upon affordable care management plan is established that allows for changing circumstances and individual needs. Once designed and agreed upon, Comfort Connection implements and monitors the care plan. Comprehensive, real-time feedback communication is provided to designated family members, whether they reside in the next town or across the country, providing unparalleled peace of mind.

When a transition to alternate living arrangements is preferred, we help identify appropriate care setting options, which are presented to the family to assist in ensuring a smooth and timely transition.
